New endpoints

• Visit recurrence should-know templates — list and read
• Visit recurrence must-do templates — list and read
• Update a worker rate

New fields

• appointed_decision_makers on the client resource, each with authority_scope and expires_on
• registered_supporters on the client resource, each with status, conflict_of_interest, conflict_of_interest_details, identifier and registered_on
• appointed_decision_maker and registered_supporter in the roles returned for a profile
• template_id on the must-do and should-know resources on visits, so consumers can tell which template each one came from
